The drawing for the Miracle Home Makeover will be held on Wednesday, October 21, 2015, at 6pm at FSLH's St. Luke's Campus, New Hartford, and will be audited by an independent auditing firm.
A maximum of 4,300 tickets are available and may be purchased for $100 each. The FSLH Foundation is working with long-time CMN corporate supporters First Source Federal Credit Union, GPO Federal Credit Union and Chanatry's SuperMarket to have tickets available at the following locations:

Faxton St. Luke’s Healthcare (FSLH) Foundation, in partnership with WKTV NewsChannel 2, Lite 98.7FM, The Observer-Dispatch and Lewis Custom Homes, is fulfilling dreams with the Miracle Home Makeover to benefit the children of the Mohawk Valley.
Miracle Home Makeover Terms and Conditions: This Miracle Home Makeover raffle ticket is offered by the Faxton St. Luke’s Healthcare Foundation, a New York State not-for-profit corporation qualified under Sec. 501(c)(3) of the Internal Revenue Code to benefit our Children’s Miracle Network Hospital (CMN). Ticket price $100. Your payment may be made by cash or check payable to Children’s Miracle Network or funds credited to CMN against a donor’s MasterCard, Visa or Discover charge account. If your entry ticket is received by Wednesday, October 21, 2015, prior to 5pm, Faxton St. Luke’s Healthcare Foundation will enter the stub on the face of this ticket into the Miracle Home Makeover raffle. The drawing for the Miracle Home Makeover will be held at 6pm on Wednesday, October 21, 2015, at the St. Luke’s Campus of FSLH, 1656 Champlin Avenue, New Hartford. Donor need not be present to win. The winner of the raffle will be selected at random from eligible entries and will be notified by certified mail to the address given on the stub within 10 business days after the drawing. The winner must present this ticket and identification to claim the prize and has until 5pm on the fifth business day after the postmark of the mailed notice to do so. If the prize is not so claimed by the deadline, the next drawing ticket may be awarded the prize, subject to presentation of that ticket and identification by 5pm on the fifth business day beginning with the business day after the postmark of the mailed notice to such party. Copies of these terms may be obtained by mailing a stamped, self-addressed, return envelope along with a request for such a copy or copies to Faxton St. Luke’s Healthcare Foundation, Faxton Campus, 1676 Sunset Avenue, Utica, New York 13502-5475. Requests to purchase tickets by mail for the Miracle Home Makeover must be postmarked no later than Wednesday, October 14, 2015, and received prior to the drawing. The maximum number of tickets issued will be 4,300 and requests for tickets will be processed in the order in which they are received. After October 21, 2015, to receive the number of the winning ticket please send a self-addressed, stamped envelope to Faxton St. Luke’s Healthcare Foundation, Faxton Campus, 1676 Sunset Avenue, Utica, New York 13502-5475 with your request. By entering the Miracle Home Makeover raffle, the winner agrees to have their name and ticket number publicized. The Miracle Home Makeover prize to be awarded to the winner is the Makeover project valued at $100,000 in products and services from the Miracle Home Makeover participants, net of taxes, or the cash equivalent, net of taxes. The Makeover project specifications, timeframe, location and scope of work are to be approved by Faxton St. Luke’s Healthcare Foundation and Lewis Custom Homes, Inc. The winner of the Miracle Home Makeover must start the Makeover project within six months from the date of the drawing and complete the Makeover project within one year of the date of drawing. The Miracle Home Makeover will be constructed with funds supplied by the Faxton St. Luke’s Healthcare Foundation from proceeds of this drawing. The remaining proceeds will benefit our Children’s Miracle Network Hospital, supporting Women’s and Children’s Services and equipment at Faxton St. Luke’s Healthcare. The maximum number of tickets issued will be 4,300; the odds of winning will be determined by the number of entries received. Entrants must be at least 18 years of age. The Faxton St. Luke’s Healthcare Foundation staff, Foundation Board of Directors and its members, Mohawk Valley Health System Board of Directors and senior management staff, and owners of Lewis Custom Homes, Inc. and immediate families of the owners of Lewis Custom Homes, Inc. (grandparents, parents, spouses and children) are not eligible to win. The winner of the Miracle Home Makeover is responsible for all applicable taxes, specifically including withholding taxes.
